The Miraculous medal was personally designed by Our Lady of the Miraculous Medal, also known as Our Lady of Graces, when she appeared to Saint Catherine Laboure on 27 November 1830. Feast day on 27 November. 


The front side depicts Mary Immaculate with rays shining from her hands, "in all her perfect beauty". Mary stands on a globe as Queen of Heaven and Earth while her feet crush Satan and his followers. Words around the oval image say "O Mary, conceived without sin, pray for us who have recourse to thee" supporting the dogma of the Immaculate Conception of Mary.


The back side shows a cross with a bar surmounting the letter M and the Sacred Heart of Jesus and Immaculate Heart of Mary underneath symbolize the relationship of Jesus and Mary to the suffering, passion, and death of Jesus at the foot of the cross. Surrounded by 12 stars which symbolize the passage from Revelation 12:1 “a great sign appeared in heaven, a woman clothed with the sun, and the moon under her feet, and on her head a crown of 12 stars.”
